Where and When:
Tryouts will be held at Desert Ridge High School on Saturday August 17, 2013. It will be from 7-9 pm. Please arrive 30 minutes early to register.
Bring:
-A copy of the athletes birth certificate,
-Proof
of paid one day membership ($10) or your 2013 full membership (if you
played club last season) to be eligible to tryout.
If you were a USAV member during the 2012-2013 season, please bring proof of membership.
** IMPORTANT: If you were not a USAV member last season you must go on the WEBPOINT REGISTRATION SITE and register for a one day MEMBERSHIP. There is a $10 fee paid directly to the region. Bring a copy of the receipt to tryouts**
-Complete a USAV Medical Release Form
-Complete a Concussion Acknowledgment Form
IMPORTANT: All members will renew membership for the 2013-2014 season on September 1, 2013. A $50 annual registration fee will be due to the Arizona Region at that time. In addition, a 2013-2014 medical release form, 2013-2014 concussion form, and a 2013-2014 safe sport form will need to be completed.
******Roster spots will be given at the conclusion of tryouts. The start up fee of $250 will be due at this time as well as a signed player/parent commitment form.*********
